Technical issues and communication failure between the Operations Control Centre (OCC) and the hostler (driver) were among the causes that led to the collision between two Light Rail Transit (LRT) trains on the Kelana Jaya line on May 24, said Transport Minister Datuk Seri Dr Wee Ka Siong.
